CHART NOTE

Followup examination following gastroscopy.  The patient said that his symptoms were improving.  He should continue to take his Zantac 150 mg t.i.d., Reglan 10 mg q.i.d., and bethanechol 25 mg q.i.d.  He can also take antacids p.r.n.
