CHART NOTE

HISTORY
Patient is a previously healthy 62-year-old gentleman who presented for elective cosmetic removal of 2 skin lesions.  One asymptomatic wart-like lesion of a 1-cm size on the medial aspect of his right knee was removed and turned out to be benign.  The second lesion, a growing freckle approximately 2.5 x 1.0 cm on the anterior aspect of his chest, was removed and revealed malignant melanoma, Clark level III.  Patient was referred for treatment and wide margin resection.
