CHART NOTE

HISTORY
Patient was diagnosed to have new-onset atrial fibrillation approximately 8 weeks ago.  He was placed on Lanoxin followed by quinidine therapy, and several attempts were made medically to convert the rhythm to sinus without success.  Echocardiogram obtained at that time revealed dilated left atrium and
right-sided cardiac chambers.

The decision now is to hospitalize the patient for elective cardioversion and to perform cardiac catheterization to rule out congenital heart disease.  His medications at time of admission include Lanoxicaps 0.3 mg daily and Quinidex 1 tablet q.8h.
